Spruce Health processes SMS communications on our behalf and is subject to data protection obligations consistent with applicable law. SMS Text Messaging Terms of Service

These SMS Text Messaging Terms of Service (“SMS Terms”) govern the SMS text messaging program operated by Faucher Direct Care PC (“FAUCHER DIRECT CARE,” “we,” or “us”) using Spruce Health as our SMS platform provider. These SMS Terms are incorporated into all agreements between you and FAUCHER DIRECT CARE, including any agreements that are pre-existing and any agreements enacted contemporaneously with these SMS Terms.

Program Description

FAUCHER DIRECT CARE may use SMS text messaging to communicate with you regarding matters related to the services we provide, including but not limited to: appointment reminders, appointment changes or cancellations, scheduling updates, account or care-related notifications, and administrative messages. Message frequency may vary.

Consent and Opt-In

You will only receive SMS text messages from FAUCHER DIRECT CARE after providing your separate, affirmative consent to opt in. Consent to receive SMS messages is never a condition of receiving care or services from FAUCHER DIRECT CARE. SMS opt-in and consent data will not be shared with any third parties for marketing or promotional purposes.

Costs

Message and data rates may apply based on your mobile carrier and plan. FAUCHER DIRECT CARE does not charge separately for SMS communications; however, your mobile carrier’s standard messaging and data rates may apply.

Opt-Out

You may opt out of receiving SMS text messages from FAUCHER DIRECT CARE at any time by replying STOP to any message received from us at (508) 276-7109. After opting out, you will receive a single confirmation message confirming that you have been unsubscribed. Following that confirmation, no further SMS messages will be sent to you unless you affirmatively re-enroll.
